I'll break the pattern by posting that while ASC showed Efx Refract as activated, it wasn't installed and there was no install button.

I had to download and install it manually.

It took several tries to get the Activate button in the ASC to actually activate, then it took a while after it was activated before the Install button showed up. I assume ACS was downloading the file somewhere in there.

I thought the same, but when I clicked on the big box to open the right side panel, the Install button was there. It remembered that I had already tried to activate it, so when I clicked on Activate, it did, and I still had 4 activations remaining.
